So a quick recap: near the end of ep 4, Natsume (the super strong fire user that everyone fears) blasted Mikan at his highest power level, at point blank range, and she came through it without a scratch. Anyone else would have been killed.



Mr. Narumi explained to Mikan what had just happened. Hotaru, Ruka and the Iincho heard him. Clearly it isn't a secret.
Hotaru won't tell anyone because Hotaru never talks to anyone about anything. But the Iincho (his name is Yuu but no one calls him that) will certainly be asked by others in the class about Mikan's Alice. And he has good reason to tell them, because he wants Mikan to be accepted. And Ruka will talk, too. In particular, he'll tell Natsume. (Mr. Narumi had already knocked Natsume out before offering the explanation.)
Taking Natsume's hottest blast without harm is noteworthy, so Mikan is going to be on the rumor mill, and soon everyone in the school will know about her, and her nullification Alice. Inevitably some people will disbelieve and will try her out. I'm sure that the telekinetic boy who was dancing her around in the air in ep 3 will try it again. But now that Mikan knows what she can do, and has some idea of how she does it, I think she'll be able to protect herself next time. And word will eventually get around that she's the real thing.
As to Natsume, the fact that Mikan is immune to his power is very interesting. A kid with the kind of power he's got (the type and the level) is always going to wonder about the people around him. If they're being friendly, is it because they really like him, or because they're terrified of him? The only person Natsume has been sure of is Ruka, who's been his friend since they were both little. With everyone else, there's always going to be that nagging doubt. And the more fawning and obsequious the friend, the more Natsume will suspect that fear is driving it, not affection.
Because Mikan is immune to his power, and because they both know it, then it means she doesn't need to fear him, and he knows that she doesn't. So if she starts being friendly to him, he will feel more confidence that it's genuine, and be more willing to take a chance and respond. I think that's how it's going to turn out. And that's why, even if Mikan's power only directly protects she herself, it still permits her to help others.
One final thought: Mr. Narumi is a cold hearted bastard. The real purpose of his one week trial was to get Natsume to try to attack Mikan, in order to find out if she'd survive it. Narumi thought she would because he was pretty sure she had the nullification Alice, but if he'd been wrong she'd have been killed or horribly maimed. He really does remind me of Xellos in a lot of ways.
